We accept GP Chronic Condition Management Plan (GPCCMP) referrals. Referrals need to be made out to Brisbane Feeding Clinic and must include a minimum of 2 x Occupational Therapy sessions and 2 x Speech Pathology sessions. Adding 1 x Dietician session may be beneficial for patients who do not currently have a dietician.

We also often send patients back for blood tests – iron and general nutrition.  If your patient has not had a recent blood test, we would appreciate you arranging this at the same time as completing the GPCCMP.
